2024 Energy Rebate Claim Form
Beat the heat ! Thanks to the generous grant from Queensland Community Foundation, Cystic Fibrosis Queensland will be providing current financial members a once off $100 energy bill rebate.
To claim please provide a copy of the front page of your energy bill, showing a bill period covering the last quarter of the 2023 as evidence of your claim.
Applicants must be a holder of a healthcare card, disability card or unemployment benefits card to be eligible. Evidence of this card must be submitted with the claim.
Funding is limited to 40 members of Cystic Fibrosis Queensland.
Limit one per household.
Cystic Fibrosis Queensland members have until Wednesday 6 March 2024 to submit their 2024 Energy rebate claim.

• Applicants must be a current Queensland member of Cystic Fibrosis Queensland and have a diagnosis of cystic fibrosis or be a parent of a child with a cystic fibrosis diagnosis.
• A complete application form must be provided along with evidence of the expense being claimed.
